Our forms help us gather vital information to better support you and ensure a smooth setup process.

Create an account.

Once we receive your form, we will send you a link to create an account on our client portal system, SimplePractice, either via email or phone.

Step 3

Schedule an appointment.

After creating an account and verifying your insurance, you are free to schedule an appointment with one of our available practitioners.

What insurance do you take?

We take most insurances including; Medicare, Medicaid, Blue Cross Blue Shield, Aetna, HAP, and Tri-Care. We also take Private Pay, Sliding Scale, Cash Pay, and EAP. Some insurances may not be listed.

Who is eligible for service?

We offer services for the following:

  • Individuals aged 5 - 99 experiencing mental health challenges (e.g. anxiety, depression, trauma, personality disorder)

  • Families and couples seeking relationship therapy

  • Underserved populations, including low-income individuals and minority groups

  • Court-ordered individuals
